Birmingham Hip Resurfacing - Take the Next

If you are interested in pursuing

initial assessment of your medical condition and x-rays without charge.
Please follow the steps below:

- Print and complete the
Medical
History Questionnaire and
HIPAA Privacy Form (PDF
files)
- Gather recent x-rays.
To accurately assess your hip condition, we need two x-rays
of the affected hip:
-
A standing AP pelvis and
- Either a cross table lateral hip or a frog
lateral hip.
- Mail or FedEx your two forms
and x-rays to:

Dr. Rogerson, Renee
Burke, PA-C, or Joanna Hutchins,
PA-C, will call you within one business week upon receiving your
information to let you know if you are a good candidate for the hip
resurfacing procedure.
